General intake
Collect core patient details and medical history when the clinic still needs them. Patients do not repeat it for every visit.
Connect general intake, treatment-specific consent and repeat check-ins to the booking. Patients receive one branded link, sign on any device and leave the clinic with a fixed copy of exactly what they completed.

Start with AI, start from nothing or adapt the workflow around your own clinical requirements.
nuemed selects and sequences the forms around the patient and booked treatment.
Digital consent forms collect treatment-specific information and signatures electronically instead of relying on loose paper or generic PDFs. In nuemed, the form is connected to the booked treatment and the signed version is retained with the patient record.
nuemed separates general intake forms, treatment-specific consent and repeat check-in forms. The system sends only the forms required for that patient and appointment.
Yes. Add the treatment and clinical context, generate a starting form with AI, then edit and approve it. Clinics can also build every form themselves.
Yes. The form builder lets clinics create their own questions, reorder fields and use elements such as typed names, drawn signatures and checkboxes.
Yes. An answer can reveal the next relevant field. For example, selecting yes or no can open a box asking the patient to provide more detail.
No. nuemed checks what the patient has already completed. A returning patient may receive only a check-in form when that is all the appointment requires.
nuemed creates a clinic-branded PDF containing the clinic name and logo. The PDF is retained with its signature time and form version and cannot be rewritten by later template changes.
Yes. A copy can be kept for the clinic and provided to the patient as part of the clinic’s workflow.
Yes. If consent was already collected on paper, upload the signed form to the patient and appointment record. That paper form becomes the consent record for the booking, so the patient does not also need to complete the digital version.
Treatment consent and check-in forms belong to the booked appointment workflow. A general intake form may be handled separately when core patient information is required.
